Dignity Health, YRMC and CAFMA Team up for Community Paramedic Program

Prescott Valley – Dignity Health, Yavapai Regional Medical Center is honored to partner with Central Arizona Fire and Medical Authority to launch the Community Health Partnership. The partnership will provide a Community Paramedic to better serve our communities’ healthcare needs. It is not uncommon for a patient discharged from the hospital to face challenges with self-care, follow-up care, and/or support once they return home.

These challenges can result in readmission to the hospital as the patient’s health continues to decline. The goal of the new program is to reduce the number of readmissions through personalized follow-up and preventative care, while at the same time reducing the strain on the area’s health care system. The enrollment process for the Community Paramedicine Program occurs when the patient is discharged from Yavapai Regional Medical Center.

The Community Paramedicine program will be a great resource for those who need a little help after returning home from a hospital stay. This is the first program of its kind in the Quad-City area and demonstrates the strong partnership between Dignity Health, YRMC, and the Central Arizona Fire and Medical Authority. This innovative approach to alternative healthcare delivery will strengthen our community and build new relationships.
